Hi! Please make sure that you fill out all the forms correctly. If your card doesn't have a revision number just leave 00, but please notice that most cards do have a revision number. One way you can obtain the card id and the revision number is by running the command lspci for pci devices and lsusb for usb devices. Thank You!
